Talk to Malena in Gerudo Town to start the quest.
Walkthrough
Malena wanders around Gerudo Town, talking to the various guards in hopes to get them to help out her sick husband. Her husband is very ill and needs the innards of a Molduga to survive. At 6am, Malena will head to the Barracks to speak with Babi to get some help. After 2pm, she makes her way over to Reeza, the guard that blocks the entrance to Riju. After 10pm, she heads over to the Hotel Oasis to go to bed. Speaking to her at any point will begin the quest.
The goal is to go kill a Molduga and acquire some Molduga Guts. Moldugas are overworld bosses found throughout the Gerudo Desert.
