Talk to Malena in Gerudo Town to start the quest.

Walkthrough

Malena wanders around Gerudo Town, talking to the various guards in hopes to get them to help out her sick husband. Her husband is very ill and needs the innards of a Molduga to survive. At 6am, Malena will head to the Barracks to speak with Babi to get some help. After 2pm, she makes her way over to Reeza, the guard that blocks the entrance to Riju. After 10pm, she heads over to the Hotel Oasis to go to bed. Speaking to her at any point will begin the quest.

The goal is to go kill a Molduga and acquire some Molduga Guts. Moldugas are overworld bosses found throughout the Gerudo Desert. Malena will point Link towards the Toruma Dunes, found way to the west of Gerudo Town. To defeat one, stay off the sand and toss a spherical Remote Bomb. The Molduga will swallow it, then Link can detonate the bomb. Run over and swing a weapon to do some damage and then run back off the sand before it awakes. Repeat this process until Molduga has been defeated.

Return to Gerudo Town and give the Molduga Guts to Malena. As a thanks for helping her out, she will reward Link with a Gold Rupee.
