The Water Medallion is received after completing the Water Temple and defeating the boss Morpha. Ruto and Link will appear in the same room where he's met previous sages. Ruto then jokes around about being Link's wife and finally awards him with the Water Medallion. Once this cutscene is complete, Navi will tell Link to go to Kakariko Village, where he will start his quest for the next Medallion, the Shadow Medallion.
