Tears of the Kingdom
There are multiple instances of a Tattered Notebook in Tears of the Kingdom, written by different characters.
Kakariko Village Well

A Tattered Notebook can be found within the Kakariko Village Well. It contains recipes created by Koko's Mom, and her feelings for her husband and children.
Tobio's Hollow Cave

A Tattered Notebook can be found in Tobio's Hollow Cave. Reading the notebook will begin the Legend of the Soaring Spear shrine quest. The notebook gives Link a hint that he must fuse a Zonaite Spear with a Keese Wing to create a Soaring Spear. Link can then toss the Soaring Spear from the pedestal through the target to get the Utojis Shrine to appear and complete the quest.
