The Lens of Truth allows Link to see invisible objects in the game. It shows things like ghosts or hidden ladders at the expense of magic and should therefore not to be used a lot or non-stop.
Ocarina of Time
The Lens of Truth is found in the Bottom of the Well.
Majora's Mask
In Majora's Mask the lens of truth is found in the cave east of the Goron Shrine in the Goron Village. Link is helped by Kaepora Gaebora who shows him the way.
