The Fire Medallion is the Medallion you receive after completing the Fire Temple in Ocarina of Time. Once you defeat Volvagia, Darunia will appear and will tell you that he is thankful for helping him. Afterwards he will award you with the long awaited Fire Medallion. After you get this you will go to Ice Cavern.
