The Zora's Sapphire is the last of the three Spiritual Stones need to open the Temple of Time in Ocarina of Time. Ruto gives it to you after you have saved her from Jabu-Jabu's Belly. To the Zoras, this sapphire is the equivalent of an engagement ring. Therefore the only reason Ruto gives Link the item is because she likes him.
