Nominally, it is used to unlock the house with the robot on the top, by showing the medal to Baris. In there, Link can find the Phonograph and the last Piece of Heart in the game. However, if Link collects the 130 figurines available before fighting Vaati for the first time, Baris will act as if Link has the Carlov Medal and allow him to access his house. Nonetheless, it only shows in the inventory after collecting the final six figurines, including the three Vaati forms, replacing Mysterious Shells.
