Five-Star Isles is a set of small islands at the southeast corner of the Great Sea in The Wind Waker.
Heart Pieces
South of the island is a submarine, where Link must defeat several enemies. Once they have been defeated, a treasure chest appears on a raised platform. A Piece of Heart is found within it after climbing the ladder.
Using Treasure Chart #33 (which is received from Minenco on Windfall Island after showing her a picture of herself), Link can pull up a treasure chest in this area containing another Piece of Heart.
Ghost Ship
The Ghost Ship appears during the waxing crescent moon phase.
Submarine
There is a Submarine located Southeast of this island. If Link defeats the two Blue Bokoblins and the two Green Bokoblins, he can acquire a Piece of Heart.
