The Sage's Medallions are a set of key items that appear in Ocarina of Time.
Overview
The Sages' Medallions are physical tokens that symbolize the awakening and power of their respective Sage, as well as their bond to the Hero of Time. Each one is a large, metal token with the corresponding color and symbol of their Sage, and the Triforce is emblazoned on the back. In The Legend of Zelda: Ocarina of Time 3D, the Medallions' designs have been updated, to feature an ancient pattern around the rim of the Medallion.
The Sages' Medallions serve as the key items of the adult era, with each one being rewarded to Link after he has completed one of the Temples around Hyrule and awakened the respective Sage. Once Link has obtained all six Medallions, the Sages will allow Link to enter Ganon's Castle by creating a bridge over the moat.

The Light Medallion is the first of the six Medallions. It is yellow, with a symbol of three triangles pointing towards the center and three circles between them. It is given to Link by Rauru after he has pulled the Master Sword from the Pedestal of Time,OoT and awakened as the Hero of Time.OoTOoTOoT

These are the six unique Medallions that appear in Ocarina of Time. These are gifted to Link after he awakens one of the Sages from their respective temples. After Link has gathered all the medallions and awakened each of the Sages, the Sages then use their power to create a bridge which allows Link access to Ganon's Castle.

This is the second Medallion that Link receives in Ocarina of Time. After Link has completed the Forest Temple he awakens the Forest Sage Saria. She then gifts Link with the Forest Medallion. Earlier in development, the Forest Medallion was originally known as the Wind Medallion and could have been equipped and functioned very similar to Farore's Wind.
The Water Medallion is the fourth of the six Medallions. It is blue, with a symbol of six small ovals in a snowflake formation. Link receives the Medallion from Ruto, after he has completed the Water Temple and allowed the ice around Lake Hylia to begin thawing.OoTOoTOoTOoTOoT

The Fire Medallion is the third Medallion Link receives in the game. It is gifted to Link by Darunia after he has navigated through the Fire Temple and defeated the evil within, Volvagia.
The Shadow Medallion is the fifth of the six Medallions. It is purple, with a symbol of an upside-down triangle surrounded by three spheres. Link receives the Medallion from Impa, after he has completed the Shadow Temple and sealed Bongo Bongo away once again.OoTOoT

The fourth Medallion that Link receives is the Water Medallion. It is given to Link after he has completed the Water Temple, by the Sage of Water Ruto.
The Spirit Medallion is the final of the six Medallions. It is orange, with a symbol of two meteors orbiting one another. Link receives the Medallion from Nabooru after he has completed the Spirit Temple and saved her from Twinrova.OoTOoT

Trivia
*Originally in development, the Sages' Medallions could have been equipped and used as regular items; for example, the Forest Medallion was originally named the Wind Medallion, and would have functioned similarly to Farore's Wind.
Shadow Medallion
*The Forest Medallion's symbol resembles that of the Bombos Medallion from A Link to the Past. In Ocarina of Time 3D, the appearance is altered to have three crescents instead of four, likely to avoid resemblance to a swastika.
This is the penultimate Medallion that Link receives in Ocarina of Time. It is gifted to him by the Sage of Shadow Impa after he has completed the Shadow Temple.
*The Spirit Medallion's symbol is based on the sign of Cancer, or the Yin and yang symbol.
*The emblems of the Sages' Medallions appear around the Arbiter's Grounds in Twilight Princess, and in the ceiling of the  Sealed Temple in Skyward Sword, after the Statue of the Goddess has returned to the Surface.
