The Moon's Tear is only found in Majora's Mask. It is what the name suggests: a shiny rock that falls from the moon's eye. In Majora's Mask, the moon has a face. As Deku Link, Link can trade it with the Business Scrub in exchange for the use of his Deku Flower. This allows him to get to the entrance of the Clock Tower, which is where he can find Skull Kid.
